Use this form to request assistance obtaining Slovak documents for your anchor ancestor, including birth (Rodný list), marriage (Sobášny list), and death (Úmrtný list) certificates.

You must provide complete and accurate information for us to proceed. All information requested applies to the CBD/SLA applicant and their ancestors. If completing the form on someone else's behalf, list their information, not yours. Submit one form per ancestor.

You will receive a bilingual authorization form allowing us to request documents on your behalf. This form must be notarized, scanned, and emailed back to us before we can submit requests.

Fees & Payment:

  • Service fees are prepaid and non-refundable.
  • We make every effort to obtain records, but archives may issue a negative response due to missing, incorrect, lost, or destroyed records.
  • If a record cannot be located, we will provide proof of the request and a negative response.
  • Document fee: The rates below INCLUDE the cost of one copy. Extra copies start at 7€ each, depending on the document.

Service Fees (per ancestor):

  • €60 – first document from an archive
  • €35 – each additional document from the same archive (for a different ancestor)

Separate archive requests:

  • €60 – first document
  • €35 – each additional document (different ancestor)

Examples: 

  • Two ancestors, same village/timeframe/year/archive: €95
  • Two copies of the same record for the same person: €67
  • Two ancestors, same village but different archives (generally due to different years. For example, an 1894 record held at a state-level archive, a 1900 record at a regional archive, and a 1920+ record at the local registrar. These are three different locations.) : €120 

Additional Costs:

  • Shipping: €67 via DHL
    • Shipping can be avoided if using Iveta ÄŒerveňaková for translations; documents will be bundled and shipped once translations are completed, and you will pay shipping only once.

Processing Time:

  • Allow up to 2 weeks for us to begin work.
  • Archives have 30 days to respond from the time they receive our request.
  • Registry offices may take an additional 30 days to issue the document once they receive the extract from the archive.
  • We will contact you upon receiving a response. Please allow 90 days for processing.

Important Notes

  1. We are not genealogists. We can only request documents based on accurate information you provide. If you need genealogical research, we can refer you to a qualified Slovak genealogist.
  2. For CSR passport requests: Only for post 1918 arrivals, and you must provide the year they traveled in the notes section below. Many records were destroyed in the war, and some years, including 1920, are not available.
  3. For Confirmation of Nationality - This requires having located your ancestor on the 1930 CSR census. If you are requesting this document, please include the link to their census sheet in the notes section below.

What is the applicant's phone number? Please use digits only. Do not include +, -, etc. If you are in the US or Canada, do not include "1". Please only include the country code if you are outside the US or Canada.

This should be an email also associated with a Google account.

Complete address, including city, state, and zip code.

Do not include Americanized, altered, or former Hungarian village names. 

Only list the original name used at the time of birth and the present-day Slovak name of the city/town/village.

Required details for each record request, provide: Full birth name(s) Exact date(s) (spell out the month) - Type of event (birth or marriage) -  Place of event (present-day Slovak town/village) - Ensure all information matches the bilingual authorization form.

If you are requesting documents for multiple ancestors, please specify whose parent you are listing. Use only the names they were born with. For example, John Doe's parents are Dan Smith and Jane Random, and Mary Joe's parents are Bob Clark and Mary Hart.
