Citizenship by Descent and Slovak Living Abroad  Client Intake Form

💼 Our full-service consulting fee: 

1200€ for Citizenship by Descent (CBD)

-or-

1200€ for the Slovak Living Abroad Certificate (SLA)

-or-

2000€ for both SLA & CBD

This includes comprehensive, step-by-step guidance—from your first question to being fully prepared to submit your application. The fee covers the entire family - you, your parents, siblings, cousins, and children. However, we deal only with the client. Family is welcome to share resources, but we can only deal directly with the client. 

📝Client Intake: Step-by-Step Guide

1. Complete the Intake Form Below

• Fill out the short intake form below.
• Use an email address linked to a Google account to ensure seamless access to your shared folder.

2. Schedule Your Intake Call

• Use the scheduling link at the end of the form to self-schedule your 15-minute intake call. 
• Both the form submission and appointment scheduling are required before you can be contacted.

3. Wait for Your Shared Google Folder

After we receive your form and appointment request, you will receive access to a shared Google folder within 24 hours containing:

• Your lineage chart
• A bilingual authorization form (permits us to request your ancestral Slovak records)
• Our consulting contract for review only. Please review this ahead of your intake appointment so that you are familiar with the services being offered. An official contract will be sent to you via EuroSign (similar to DocuSign) for official signature after your intake call.

4. Complete the Required Documents

• Fill out the lineage chart.
• Complete the bilingual authorization form and have it notarized.
• Review (but do not sign) the consulting contract; a digital version will be sent for signature after your intake call appointment.

5. IMPORTANT! Complete the Lineage Chart on Time

• Your appointment will be canceled if the lineage chart is not completed at least 24 hours before your scheduled call.
• This chart is essential for confirming your eligibility before we accept you as a client.

6. Prepare for Payment

• The contract will be signed, and payment will be made after your 15-minute intake call.
• Payments are made through Wise, a service for international transfers and global currency transactions.
