LĐBĐ ACTIVITY CONSENT FORM AND APPROVAL BY PARTICIPANT, PARENT, OR LEGAL GUARDIAN

The participant(s) named above has approval to participate in (name of activity, orientation, flight, outing, trip, etc..) 

Liên Đoàn Biển Đông Activities and / or Trip.   From 11/1/2025 to 12/31/2026.

I understand that participation in Hướng Ðạo activities involves a certain degree of risk and can be physically, mentally, and emotionally demanding. Including each activity in my immersive activities, even  of those risks arise from the negligence or carelessness of the host or others, or defects in the  equipment, premises, or facilities used during the activity, or otherwise, and I assume full responsibility  for participation in the activity. I have carefully considered the risk involved and have given consent for  myself or my child to participate in this activity. I also understand that participation in this activity is  entirely voluntary and requires participants to abide by applicable rules and standards of conduct.

I release the Hướng Đạo Việt Nam, Liên Đoàn Biển Đông, the activity coordinators, land owner host, and  all employees, volunteers, related parties, or other organizations associated with the activity from any and  all claims or liability arising out of this participation.  To the maximum extent permitted by law, hosts  provide the activities and/or trip "as is," without warranties or any kind, whether express or implied. Without  limiting the foregoing and to the maximum extent permitted by law, host(s) expressly disclaim  any warranties of safety, fitness for a particular purpose, quiet enjoyment, and as to the adequacy of the   directions and warning provided by me.

I agree that if I or anyone on my behalf make a claim against the Host(s) relating to any activity and/or Trip, I will indemnify and hold the Host(s) harmless from any liability, demand, loss, damage, or costs which the  Host(s) may incur as the result of such claim.

When activities take place on private land, it is an understanding that I treat the land and its owner with respect, in exchange for the privilege of use of the land for camping and activity. I adhere strictly to any rules set by the landowner regarding where, when, and how I can carry out the activity. Practice the "Leave No Trace": Pack out all your trash, including empty cartridges, and minimize your impact on the land. Leave all gates as I found them, stay in designated areas, and avoid damaging crops or planted fields. If I notice any issues, like damage to a fence, report it to the landowner immediately. In case of emergency involving my child, I understand every effort will be made to contact me.

In the event I cannot be reached, I hereby give my permission to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections  of medication for my child. Medical providers are authorized to disclose to the adult in charge examination 
findings, test results, and treatment provided for purposes of medical evaluation of the participant, follow up 
and communication with the participant's parents or guardian, and/or determination of the participant's 
ability to continue in the program activities.
